The COBRA Community Follow-Up Program (COBRA) is an expansion of the comprehensive scope of AIDS-related services offered at Harlem United.
COBRA provides bilingual, intensive, family-centered, case management services to HIV-infected individuals and high-risk persons. COBRA clients have experienced difficulties or challenges in the past to accessing medical care and/or other services. They may require frequent personal contacts and/or home visitation to ensure their continued connection to medical care and other needed services.
Harlem United’s COBRA program promotes:
• access to HIV and STD information, counseling, and testing;
• access to social, mental health, or substance use services to prevent or delay institutionalization or hospitalization;
• continuity of healthcare and appropriate follow-up;
• access to social services to enhance client independence and self-sufficiency;
• access to appropriate additional services to support the client and the client’s family;
• coordination among service providers to eliminate duplication of services.
