Transitional Housing

The Transitional Housing Program aspires to assist persons to acquire independent living skills, to participate in a model of intervention that promotes disease management through connection to care, harm reduction, recovery readiness and relapse prevention, addiction education, reduction of high risk behavior and ultimately to increase one’s quality of life through the provision of high quality care. The Transitional Housing Program aims to foster Harlem United’s mission to provide one hundred percent access to care and obtain zero disparity in health outcomes caused by racial and economic barriers.

The Transitional Housing Program services include:

  • Weekly Case Management Home Visits

  • Primary and Home Care Referrals
  • Entitlement Assistance and Advocacy
  • Linking/Referrals for Legal Assistance
  • Substance Use Assessment and Counseling
  • Mental Health Assessment and Counseling
  • Medical Monitoring and Medication Adherence
  • Life Skills Training/Education
  • Apartment Unit Maintenance/Environmental Safety Support
  • Housing Referral, Placement and Aftercare
