HOUSING

How do I get housing from Harlem United?

Harlem United's Housing programs are available to persons living with HIV/AIDS. Harlem United only accepts referrals for housing from NYC's Human Resource Administration's HIV/AIDS Services Administration (HASA). Questions for HASA can be directed to their ServiceLine, 212-971-0626.

COUNSELING

How do I get counseling and support from Harlem United?

Harlem United's Adult Day Health Center (ADHC) serves people living with HIV/AIDS in a friendly, stress-free, healing environment. The treatment center was established with the goal of assisting multiply-diagnosed clients adhere to complicated antiretroviral regimens while addressing other co-morbidities.

To be admitted into the healthcare program, you need to provide the following:

  • A referral from a Doctor with written confirmation of HIV status, usually in the form of an M-11q or its equivalent form coming from the New York State Department of Corrections,
  • Your most recent PPD testing status,
  • A copy of your most recent Chest X-ray and
  • Active Medical Insurance: Medicaid

What is Medicaid?

Medicaid is a program for New Yorkers who can't afford to pay for medical care.

How do I know if I qualify for Medicaid?

You may be covered by Medicaid if:

  • You have high medical bills.
  • You receive Supplemental Security Income (SSI).
  • You meet certain financial requirements.

How do I apply for Medicaid?

You can apply for Medicaid in any one of the following ways: Write, phone, or go to your local department of social services. You can also mail in an Access NY Health Care application to your local department of social services, or apply with the help of an enrollment facilitator.

In New York City, contact the Human Resources Administration (HRA) by calling (718) 557-1399. Residents of the five boroughs of New York City may call toll free at 1-877-472-8411.

You can also apply at many clinics and hospitals: Harlem Hospital, Metropolitan, and Mt. Sinai

To apply for Medicaid, you need to provide the following:

  • Proof of age, like a birth certificate
  • Proof of citizenship or alien status
  • Recent paycheck stubs (if you are working)
  • Proof of your income from sources like Social Security, Supplemental Security Income (SSI), Veteran's Benefits (VA), retirement
  • If you or anyone who lives with you is 65 years old or older, certified blind, or certified disabled, you need to give information on bank accounts, insurance policies and other resources
  • Proof of where you live, like a rent receipt, landlord statement, mortgage statement, or envelope from mail you received recently
  • Insurance benefit card or the policy (if you have any other health insurance)
  • Medicare Benefit Card (the red, white and blue card

HOLISTIC COMMUNITY HEALTHCARE (HCH)

What is Holistic Community Healthcare (HCH)?

HCH is an integrated harm reduction program providing services to homeless and transient populations with a history of substance use and mental health illnesses. "Homeless" is defined as anybody who does not have a lease in their name (i.e. living in shelters, on the street, in transitional housing, etc). HCH integrates Harlem United's Healthcare for the Homeless and Treatment for the Homeless programs.

What services does HCH provide?

Services provided, but are not limited to, access to healthcare, dental care, and psychiatric services; behavioral health services, individual and group counseling, case management, outreach and community organizing, and linkages/referrals to other programs.

Who is eligible for services?

HCH serves homeless and marginally housed men and women, and people with a history of substance use. Services are provided regardless of HIV status, insurance status, or immigration status.

How can I access healthcare / primary care services?

HCH and Harlem United's Primary Care clinics provide walk-in services for healthcare appointments, however you are encouraged to call and make an appointment. If you have Medicaid, please bring your card with you. If you do not have Medicaid, we will assist you in getting your benefits in place.

What if I don't need healthcare services, but need counseling or other supportive services?

HCH will provide you with individual and group counseling services to discuss topics such as substance use, harm reduction, safety from homelessness, vocational skills, life management and wellness living skills, and more.

What if I need help stying sober? What if I'm not in recovery?

HCH provides services to individuals with a history of substance use. You can be in any stage of preparation for recovery, and if you're not ready, that's fine to. We will work with you at achieving your goals, at your own pace. Our Treatment for the Homeless program will assist you with addressing how to minimize harms caused by substance use, and prepare you for a path of recovery if that's your goal.

TESTING

What tests are available?

We can provide HIV, Syphilis, Gonorrhea, Hepatitis C and Chlamydia testing.  All testing is free of charge.

What are the hours of operation?

We provide free HIV and STI testing Monday to Friday 10am to 4 pm at our 290 Lenox Avenue location.

For the summer months we have extended testing hours on Tuesdays and Wednesday 10am-630pm

Are appointments necessary?

No appointments necessary, walk-ins are welcomed.

How soon can I expect my test results?

You can obtain your Rapid HIV test results in about 20minutes, STI results can be picked up in about 1 week.

Will blood work be taken on me?

Blood tests are conducted for Syphilis and Hepatitis C testing. Urine collection is required for Chlamydia and Gonorrhea testing.

HUow long is the waiting period to be seen for testing services?

Depending on client flow, clients can expect to be seen within 20-30minutes
