

The Harlem United community AIDS Center offers people living with HIV/AIDS comprehensive, cutting edge primary and specialty medical care. Everyone receives the same HIV-sensitive, quality care, regardless of their ability to pay. We accept a variety of insurance plans and if you're uninsured and have limited financial means, our staff will help determine your eligibility for a variety of medical care and drug assistance programs and help you complete the appropriate applications.
The facility is staffed by HIV/AIDS specialists, including board-certified physicians, social services case managers, and a nutritionist. In addition to medical care, the Center's HIV/AIDS services include case management, mental health counseling, complementary therapies such as art therapy and nutritional counseling.

Harlem United provides comprehensive, quality Primary Care services to our most vulnerable populations. Our patients often have multiple diagnoses such as HIV, mental illness, substance use and chronic illnesses such as Hepatitis C and Hypertension. Our team strives to provide sensitive, compassionate, integrated care in a professional atmosphere. Upper Room AIDS Ministry was awarded a Healthcare for the Homeless Grant by HRSA effective May 2010. The grant allows us to expand our mission and serve clients who are both HIV positive and negative. The services provided are primary care, dental care, psychiatry and supportive case management. Recently, the name of the program was changed to Holistic Community Healthcare due to the merger HCH and Treatment for the Homeless merger. As a result, clients can also receive substance use pre-treatment services. The El Faro Center on East 116th Street, which is fully staffed with bilingual Spanish/English-speaking staff.

Building Bridges provides mental health services to PLWHA with mental illness or substance abuse disorders in order to increase their level of functioning and reduce various barriers to access and engagement in HIV primary medical care. The ultimate goal is to promote optimal health outcomes, thus facilitating clients’ overall well-being and quality of life from a holistic approach.

Adult Day Health Care Center West is a multi-service program designed to enable people living with HIV/AIDS to live healthier, longer, more independent lives. We address clients’ issues within a community that is responsive and caring and thus postpones or obviates the need for long-term hospitalization or residential health care. The goals of the ADHCs include the elimination of barriers to care, improvement in treatment adherence, and reduced hospitalizations. Our team strives to provide sensitive, compassionate, integrated care in a professional atmosphere. The AHDCs provide a community for traditionally marginalized individuals with minimal bio psychosocial supports outside of the ADHC Programs. The Adult Day Health Care model is premised on the hypothesis that it can level the playing field and render HIV treatment for our clients as effectively as treatment delivered to the general population. This is achieved by keeping our clients as healthy as possible while they are living in independent settings.

Our dental clinic was created in response to our clients’ need for quality dental care. (Good dental care is key to maintaining the health of people with HIV/AIDS.) Our clients requested a full-service dental clinic that was easy to access for historically underserved and special needs population. The COBRA Community Follow-Up Program (COBRA) is an expansion of the scope of AIDS-related services offered at Harlem United. COBRA provides comprehensive family-centered intensive case management services, including home visitation and advocacy in the community, for adults living with HIV/AIDS. The goals of COBRA are to increase access to HIV-related services and medical care, prevent or delay institutionalization including hospitalization, and foster independence and self-sufficiency for clients. COBRA also provides services to clients' children up to age three.

Harlem United has three mental health programs providing a wide range of services for persons living with HIV/AIDS.
Building Bridges is funded as a performance-based contract by Public Health Solutions, and is designed to provide mental health services to PLWHA with mental illness or substance abuse disorders in order to increase their level of functioning and reduce various barriers to access and engagement in HIV primary medical care. The ultimate goal is to promote optimal health outcomes, thus facilitating clients' overall well-being and quality of life
The Enhanced Mental Health II program provides individual psychotherapy to clients in need of long-term treatment. This program works in collaboration with the medical clinics at the Willis Green Health Center and the El Faro Center and is funded by the AIDS Institute.
The Mobile Mental Health program is funded by SAMHSA. It provides high-quality culturally competent, mental health services to HIV/AIDS clients who are dually or triply diagnosed. Therapy is conducted in traditional and non-traditional settings in the form of crisis/brief interventions, short-term (three months) and long-term (six months) therapy, and therapeutic groups. In order to be eligible, clients must manifest symptoms of mental health decompensation, such as isolating shifts in mood, agitation, anxiety, sadness, nervousness, sleep disturbances, suicidal thoughts or expressions, difficulty concentrating, coping with change, relationship/family conflicts, physical and/or sexual abuse and reports of domestic violence.

The Food and Nutrition Program provides comprehensive food and nutrition services including initial and on-going assessments, appropriate interventions and on-going monitoring to PLWHA. Services include individual- and group-level nutritional education, congregate meals, and pantry bags. Nutritional education provides clients with strategies for improving their nutrition based on their health status, life skills and other mediating factors.

The Harm Reduction/Recovery Readiness and Relapse Prevention Program (H4RP) serves people living with HIV/AIDS, as well as people who are HIV negative, all of whom struggle with chronic substance use, high risk behavior, homelessness and poverty. Services are generally designed for clients who are in preparation, action and maintenance, or relapse stages of recovery.

The Testing Services Department aims to identify and test at-risk populations in Upper Manhattan and the South Bronx for HIV, STIs and hepatitis C. The Testing Department offers both initial, rapid testing and follow-up confirmatory tests for HIV. We have trained phlebotomists to draw blood for STI and hepatitis C screening. We employ a variety of recruitment strategies to get people tested; including alternative venues, targeted outreach, and social networks. Testing services targets multiple populations (high-risk heterosexuals, high-risk and HIV-positive women of color, active substance users and LGBT) through New York City, New York State and CDC-Federal contracts.
