

Our Supportive Housing Division provides over 588 scattered-site and congregate units for PLWHA, including women and children, undocumented immigrants, and the mentally ill. Supportive Housing clients receive a crucial array of supportive services including primary care support, treatment education, recreation, mental health services, structured socialization, advocacy, and substance use counseling.

The goal of the Health Home program is to reduce cost while increasing quality and efficiency in New York’s Medicaid Program. The Health Home program will improve the health care provided to members of the Medicaid Program and will save money by reducing preventable hospitalizations, emergency room visits and unnecessary care. Health Homes provide a care coordination model for high-cost, high-need Medicaid recipients. It is a care management service model whereby all of the professionals involved in a member’s care communicate with one another so that all needs (medical, behavioral, health and social service) are addressed in a comprehensive manner.

The FROST’D HUD Scattered Site Housing Program provides transitional housing for HIV/AIDS positive individuals with a history of or whom are currently homeless. For a 12-24 month period the program works to ready clients for independent living and to successfully manage their illness. The apartments are located in Brooklyn neighborhoods. This program has the capacity to serve English and Spanish-speaking clients.

The NYNY III program provides 70 units of permanent scatter site housing and supportive services to chronically homeless single adults who are living with HIV/AIDS and co-occurring serious and persistent mental illness, substance abuse disorders, or both. Chronically homeless is defined as an unaccompanied disabled individual who has been continuously homeless (living on the streets or in a shelter) for at least one year. Our program works with clients to stay healthy and remain housed, using a multi-disciplinary team approach.

Foundation House North and Foundation House South provide emergency transitional, congregate housing for up to 180 days of care. Foundation House North is located in Washington Heights and has 10 rooms (4 rooms for single adults and 6 rooms for adult couples) the maximum capacity is 16 adults. Foundation House South, located in the Lower Eastside, has 14 rooms for single adults.

Brooklyn HUD Scattered Site Housing provides 32 units of transitional housing for homeless single adults living with HIV/AIDS who are HASA eligible. For a 12-24 month period, the program works to prepare residents for independent living and successfully managing their illness. The goal of the program is to graduate residents after 12 months, thereby transferring the lease into the resident's name and reducing the need for supportive housing case management services. The program is ideal for homeless adults living with HIV who are in need of short-term supportive services to stabilize their health and living conditions. Apartments are located in Bedford/Stuyvesant and surrounding Brooklyn neighborhoods. Residents must pay 30% of their income toward rent; no rent portion charged if the resident's sole income is Public Assistance.
