Care Manager –Primary Care, Health Services

The Care Manager for Primary Care has the overall day –to-day responsibility for coordinating the activities of the care team for clients with complex medical and/or psychiatric conditions and for facilitating client’s access to the full range of medical and psychosocial services in an efficient and effective manner.  The Care Manager also serves as the PrEP Navigator and is mainly responsible for conducting initial intakes and assessments for persons at high risk of contracting HIV, providing linkage to insurance, benefits and other social services while providing information on how to get PrEP prescriptions and setting up primary care appointments.  The Care Manager is responsible for coordinating medical care for patients during their ER visits or hospital admissions, and actively participating in discharge planning and care transition activities. Education and Certification: Master in Social Service preferred or Bachelor Degree in the Social Services field + 2 years of relevant experience.
