HARP Care Manager, Health Home

The HARP (Health and Recovery Plan) Care Manager has overall day-to-day responsibility for coordinating the activities of the care team for clients designated by the Department of Health as “Special Population” with complex psychiatric conditions and for facilitating clients’ access to the full range of medical and psychosocial services in an efficient and effective manner. The HARP Care Manager is mainly responsible for coordinating medical care by receiving inpatient and ER admissions of targeted clients. In addition, the HARP Care Manager is responsible for visiting clients during inpatient stays, participating actively in discharge planning and care transition activities. Furthermore, the HARP Care Manager is responsible for completing an initial and yearly HARP and a Health and Community Based services (HCBS) eligibility assessment, a HARP Plan of Care (POC), and requesting a Letter of Determination of Services from the Managed Care Organization (MCO). Together with the HARP Care Navigator, the HARP Care Manager is responsible for the delivery of services for high need clients by collaborating closely with the MCOs and HCBS providers. Education and Certificate: A bachelor’s degree in any of the following: child & family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreational therapy, rehabilitation, social work, sociology, or speech and hearing.
