Nurse Practitioner, Health Services
Under the supervision of the Medical Director and in accordance with established policies and procedures, the Nurse Practitioner delivers high quality comprehensive primary care for homeless, underserved, chronically ill populations including patients with HTN, Diabetes, Asthma, Heart Disease and other comorbidities. Experience in management of HIV, Hepatitis C and Substance use disorder is preferred. The AIDS institute Guidelines www.hivguidelines.com and the U.S. Preventive Services Task Force (USPSTF) http://www.ahrg.gov/clinic/uspstfix.htm are utilized as resources and guides for accepted standards of care. Education and Certificate: Graduate of an Accredited Nurse Practitioner Program.
