NYNY III Case Manager (MH Specialist), Housing
Harlem United Community AIDS Center, Inc. is a community-based organization in Harlem that provides integrated healthcare, prevention, and housing to people living with HIV/AIDS in Central Harlem and the South Bronx. The NYNY III Case Manager (MH Specialist) will work within a unique, multidisciplinary case management team to provide a continuum of home-based biopsychosocial services to MICA clients in a supportive housing program for PLWHA. Under the supervision of the Program Director, the NYNY III Case Manager (MH Specialist) will provide home and community-based assessment, mental health and substance abuse counseling, treatment referral, and care coordination with other team members and service providers to clients enrolled in the NYNY III program. In addition, the NYNY III Case Manager (MH Specialist) will address clients’ needs with regard to concrete housing issues as necessary. Education and Certification: BA in related field, or CASAC with at least 1 year of related case management experience or at least 5 years of field-based case management experience working directly with the HIV and substance using population.
