|As part of the Check Hep C initiative, the Patient Navigator/Outreach Worker will be responsible for the successful linkage of all HCV+ individuals identified and engaged by the program. Implementing the ARTAS intervention, the Patient Navigator will be involved working to link individuals into primary care services and other support services through internal and external referrals. In support of program enrollment, the Patient Navigator is also responsible for assisting patients in understanding liver health care in the presence of a positive Hepatitis C diagnosis, and navigating the social service and medical system in order to access necessary supportive services to support liver health, complete a liver health medical evaluation, and initiate Hepatitis C treatment if recommended by a liver specialist/infectious disease specialist. Working as part of a team, the Patient Navigator will also provide assistance escorting HCV+ individuals to our on-site location for program enrollment. Under the supervision of the Program Coordinator, he/she will be responsible for assisting peer-led case finding activities, peer-led group activities and provide navigation services to ensure successful outcomes to care. He/She will also be responsible for providing similar navigation services to co-infected individuals on a case by case basis. Education and Certificate: A minimum of a Bachelor’s degree or commensurate work experience of five years working in community health care, harm reduction, HIV prevention programming or HIV/STI testing is required.
