Patient Navigator, Prevention

The Patient Navigator is responsible for conducting HIV testing and STI screening on-site at Harlem United locations, at community-based settings and in Harlem United mobile units, throughout Harlem and other targeted locations throughout NYC. The Patient Navigator conducts rapid HIV/HCV testing and STI screenings and provides risk reduction and sex-positive counseling. They provide rapid test results, conduct confirmatory tests as needed, as well as delivering STI screening results. Conduct PCRS and refer to NYCDOHMH CNAP as needed.  The PrEP Navigator needs to be a self-starter and work efficiently in a team environment and independently.  This position requires the person to be an LGBTQ advocate, promote sex positive counseling, be comfortable in engaging persons of all gender identities/presentation and expression, sex workers, active substance users and the diverse community of NYC.Education and Certificate: Bachelor’s Degree with 2 years related work experience.  A high school diploma OR GED equivalent with 2-3 years’ experience in HIV testing and patient navigation programs will substitute for a Bachelors Degree.  Candidate must be an advocate for high- risk populations, including African American and Latino young men who have sex with men (MSM) and transgender individuals, substance users, and sex workers.  Prior risk reduction and/or health education counseling work experience, knowledge of HIV testing services and patient navigation preferred.  To Apply: Submit your resume with a cover letter indicating position and salary requirements to:
