Population Health Specialist, SQIRE

The Population Health Specialist is responsible for researching, planning, coordinating and reporting quality improvement initiatives for the Federally Qualified Health Center.  Population Health Specialist will work in collaboration with invested parties including but not limited to funders, Managed Care Organizations and senior administrator within Health Services to facilitate quality projects and outcome reporting. This position will directly oversee: collaborative/committee projects/special projects, quality assurance/quality improvement, and population health contract requirements. The Population Health Specialist will act as primary contact and help coordinate funder driven initiatives and identify care gaps within patient population. The Population Health Specialist will report to the Senior Director of SQIRE and work with evaluation staff to execute Continuous Quality Improvement projects as well as conduct bi-monthly quarterly quality assurance reports for the FQHC. Education and Certification:  Master’s Degree preferred, or Bachelor’s Degree with equivalent work experience of 1 year or more in Population Health, QI/QA activities.
