Team Supervisor, Health Home Plus

In collaboration with the Managing Director, the Health Home Plus Team Supervisor provides overall supervision to the Health Home Plus Case Managers; assist in staff recruitment/hiring, and implementation of the program policies and procedures. He/she works in collaboration with the management team and care management staff to comply with the Health Home Plus lead requirements. The Health Home Plus Team Supervisor is responsible for monitoring the implementation of the Health Home Plus core services to all enrolled clients; specifically for the implementation of the needs identified by clients during the intake, assessment, and reassessment process. The Health Home Plus Team Supervisor plays an active role in the daily supervision of the Health Home Plus care management. He/she helps the staff to prioritize and complete their schedule to maximize the provision of client’s services. Education and Certification: Master’s Degree in Health, Human Services, Mental Health, Social Work and one year of supervisory experience and one year of qualifying experience or Bachelor’s Degree in Health, Human Services, Mental Health, Social Work and two years of supervisory experience and three years of qualifying experience Excellent written communication skills; detail oriented, Experience working with people with multiple medical conditions (preferred, but not required). Knowledge/experience of Microsoft (excel), Epaces, and EClinicalWorks, RMA and GSI a plus.
