Temporary Case Manager, HOPWA

Responsible for the provision and coordination of mental and physical health services, case management, supportive counseling, follow-up, home visits, social service referrals, and outreach on behalf of clients in a Scatter Site Housing Program. Ensures compliance with contract services requirements, carries a caseload, maintains regular contact with clients, ensure client is active in program responsibilities primarily in regard to health care. This is a temporary position that will end on June 30, 2020. Education and Certification: q MA in related social service field (BA in social service field plus at least 3 years of work experience in the field or MA plus 1 year work experience in the field may be considered in replace of having a MA in the social service field). Knowledge/ background working with the HIV/AIDS population and services rendered to the HIV/AIDS, chronically homeless, and substance use population and their families.
