Harm Reduction Counselor/Outreach Worker/Driver – 2 Positions Available

 

This position is responsible for providing an array of mobile-based harm reduction services including intakes and ongoing assessments to people living with and at risk for HIV/AIDS, rapid HIV testing, syringe exchange, substance use and sexually transmitted infections (STIs) screenings and assessments, referrals, street outreach, and additional responsibilities as requested. Mobile services are conducted utilizing a team approach in the Prevention Division of Harlem United. Services are delivered to marginalized street-based populations, including active substance users, injection drug users (IDUs), people living with HIV/AIDS and/or Hepatitis C, sex workers, and other high-risk populations. Overdose education and relapse prevention is conducted on a regular basis to all clients with a history of substance use. This position is also responsible for driving the mobile units and assisting with vehicle maintenance. HIV Test Counselor

 

The HIV/STI Test Counselor conducts rapid HIV testing and STI screenings on-site at Harlem United locations, at community-based settings, including SROs, shelters, and public housing developments, and on the Harlem United testing van, throughout East and Central Harlem. Support the clinic based testing initiative at El Faro, ADHC west and dental clinic to routinize and build testing capacity at Harlem United’s medical facilities and Provide HIV/STI education and risk reduction counseling. Deliver rapid test results, conduct confirmatory test as needed as well as delivering STI screening results. Provide referral and connection to medical care/treatment, mental health services, and other social services as required. Conduct PCRS and refer to NYCDOHMH CNAP as needed. Education and Certification: Bachelors Degree preferred OR equivalent experience and HS/GED accepted. Must possess OR willing to be trained on the following:

Eligibility Specialist

 

The Eligibility Specialist screens and assist all Primary Care/Dental clients, insured or uninsured, in applying for Medicaid and other agency accepted insurance plans. Case Manager – Housing

 

The NYNY III Case Manager will work within a unique, multidisciplinary case management team to provide a continuum of home-based bio-psychosocial services to Mentally III/Chemically Addicted (MICA) clients in a scattered site, supportive housing program for People Living with HIV/AIDS (PLWHA).  Under the supervision of the Program Director, the NYNY III Case Manager will provide home and community-based assessment, mental health and substance abuse counseling, treatment referral, and care coordination with other team members and service providers to clients enrolled in the NYNY III program.  In addition, the NYNY III Case Manager will address clients’ needs with regard to concrete housing issues as necessary. Treatment Adherence Specialist

The Treatment Adherence Specialist is responsible for one-on-one and group support counseling to HIV-infected individuals aimed at increasing access to primary medical care. Working as part of our Access to Care Supportive Services team, the Treatment Adherence Specialist will work with HIV-infected individuals by providing HIV and Treatment Education and risk reduction planning.  The Treatment Adherence Specialist will work with the program Case Manager and Patient Navigators to secure and coordinate services which address the psychosocial and health care needs of HIV-infected clients, with an emphasis on removing barriers that prevent receipt of regular HIV primary care and adherence to treatment. Education and Certification: A minimum of a Bachelor’s degree or commensurate work experience of five years working in HIV prevention, programming, or HIV/STI testing is required. Program Coordinator, Patient Navigation Services

Under the direct supervision of the Program Director of Access to Care services, the Program Coordinator, Patient Navigation Services, is responsible for the successful management and implementation of our Patient Navigation program services that includes targeted case finding, mental health assessments, short-term case management, and navigation (escort and advocacy) services to homeless HIV-positive and high-risk negative individuals with mental health and/or substance use disorders. As such, the Coordinator will provide administrative oversight and support to six Patient Navigators. They will serve as the programs liaison community health partners to reduce barriers to linking clients to primary health care and access to support services. The Coordinator is responsible for supervising a team that works collaboratively to direct and navigate individuals through a continuum of care. Management and program operations are implemented using a harm reduction-based approach. Education and Certification: Master’s in Social Work, Public Health, or a related field, is preferred. Candidates may substitute a Master’s degree for 5 years experience in health care services, case management, or HIV services and prevention. Bachelor’s degree in related field is required. Data Manager, Quality Control

The Data Manager will be responsible for performing an array of activities intended to ensure the accuracy and completeness of process and outcome data collected for funder reporting, continuous quality improvement, and outcome monitoring. The Data Manager will create databases as needed, export data from NYS DOH AIDS Institute AIRS system, and work with the management team and evaluators to manage data collection tools and systems.  Relevant databases include: AIRS; eCompass; databases supporting Prevention Education and Supporting Services; and databases supporting the Foundation for Research on Sexually Transmitted Diseases; with opportunities to assist with duties as they pertain to eCW and any other databases that support Health Services. Bilingual Case Manager/Social Worker

Care Manager Supervisor, Health Home

The Care Manager Supervisor provides administrative support to the Health Home Program and is responsible for the program operation during the absence of the Managing Director. Family Counselor

Responsible for the provisions and coordination of family counseling; mental and physical health services, follow-up, home visit, social service referrals, and outreach on behalf of clients of the Women’s Housing Programs; as well as, other case management services as assigned.  Ensures compliance with contract services requirements, carries a caseload, maintains regular contact with clients, ensure client is active in program responsibilities primarily in regard to health care. Intervention Specialist

 

The Intervention Specialist is responsible for delivering two group level interventions for African American/Latina women from East and Central Harlem: 

  •WILLOW, a social-skills building and educational intervention for adult women living with HIV.

  •SHIELD: The Self-Help in Eliminating Life-threatening Diseases (SHIELD) intervention is an innovative approach to HIV prevention.

The Intervention Specialist will screen and enroll clients into each intervention and serves as the trainer/facilitator for both interventions. Additionally, the Intervention Specialist is responsible for initiating PCRS discussion with HIV positive clients enrolled into the program, and initiates a referral to the New York City - Partner Services program as required by NYC guidelines. Education and Certification: Bachelors Degree preferred OR equivalent experience may substitute a college degree. Certified Coder for Health Services

 

The Certified Coder for Health Services is responsible for maximizing revenue collection for Health Services FQHC and Dental Programs through coding training and verification, claims follow up and reconciliation. Maintain ongoing communication, collaboration, and coordination with Division Directors, staff and Executive Directors in group settings to promote understanding of revenue generation processes and quality assurance systems. Identify potential problems in eCW prior to submissions for coding errors, provider completion of progress notes, demographic and/or insurance issues through a weekly / daily quality assurance check of all visits. Ensure processes and claims submissions are compliant with all rules and regulations of the programs requirements. Reconcile all submissions against the payment remittances received and eCW for accurate postings and denial reasons. Behavioral Health Specialist

 

Under the supervision of the Program Director, the Behavioral Health Specialist (BHS) will provide comprehensive mental health and general support counseling for program members. Working in a multi-disciplinary team of Social Work Interns, and case managers, the BHS will provide individual level counseling and supportive mental health services to program members enrolled in our Youth Space program. As a member of the BMI Team, the licensed mental health worker will be knowledgeable of all the services provide by Harlem United as well as services and support available at other community based organizations and care facilities. Education and Certification: The successful candidate will have a master’s degree in social work, counseling, or psychology and at least two years post graduate proven mental health experience with difficult and hard to reach populations, (i.e. severely mentally ill, homeless, substance users, or sex workers). He or she should also be licensed. It is essential that the mental health worker has a strong knowledge of HIV, AIDS and other STD’s. Bilingual in Spanish is a plus. Contract Analyst

The Contract Analyst is responsible for publicly funded contracts/grants for the Agency in accordance with grantor mandates.  The Contracts Analyst oversees the functions of contract compliance, budget revisions, billing on and collection of public contracts. Program Coordinator, Recovery Support Services

This position will provide oversight for all aspects of the Recovery Support Services (RSS) program, a comprehensive recovery-oriented program that has a goal to offer integrated services and community support in a manner that helps individuals in their path towards recovery. The position provides supervision and oversight to a team that includes up to 5 Behavioral Health Specialists, all of whom have the expertise to provide substance abuse screenings, create Individualized Recovery Plans (IRPs) and psychosocials, as well as provide referrals, and care coordination as needed. This position will be instrumental in ensuring that all clients flow smoothly through the continuum of recovery support services, and will be responsible for overseeing  all aspects of program functioning.
